When choosing an evaluation form, remember that depending on a role, you may need to prepare several versions of the evaluation form to ensure they suit the employee. For example, there will be a different evaluation form for managers and regular employees, so there will be a different one for the IT and Marketing departments.

What to include in employee evaluation form

When preparing the employee evaluation form, you should include information to ensure the document is understandable and allows to measure employees' performance objectively.

  • Employee data - information such as employee name, job title, ID, department, and manager;
  • job responsibilities - it's excellent to list employees' duties from their job description to cross-reference them with employee performance and their progress;
  • provide descriptions - describe the goals for the given employee based on their job responsibilities and the rating system to make sure all the forms are correctly filled-up;
  • Make space for open questions and comments - they will give you more specific data to dive into when comparing the form with other evaluations;
  • Provide space to sign-up the document - for both the employee and the person conducting the evaluation.

How to write an employee evaluation form

When preparing and conducting employee evaluations, ground rules will help you gather valuable information and be a base for assisting the employees in working better, staying motivated, and feeling good in the organization's environment. Remember to:

  • explain what's the purpose of the evaluation form - to stay transparent with your employees and explain how gathered information will be helpful;
  • customize the form accordingly to each position - different responsibilities and measurements depend on the job role which must be included;
  • be objective - use the numeric scale everywhere it's possible and avoids subjective language;
  • make space for open answers - to gather more detailed information that can help prepare the professional development plan;
  • provide feedback - after evaluation, talk to employees and make sure they understand what to do to improve their job and performance.
